Job Description: The Public Health Nurse (PHN), as an integral member of the Public Health Services team, provides nursing services within the framework of the Population Health model and Primary Health Care.  The PHN works relatively independently and is expected to make primary decisions based on sound assessments in accordance with the Public Health Act, both regional and PHN specific policy and procedures, the CRNS Standards of Practice, the Canadian Nurses Association Code of Ethics for Nurses, the Community Health Nurses of Canada - Standards of Practice.  The PHN translates knowledge from the health and social sciences to support health enhancing behaviors of individuals, families and population groups through targeted interventions, programs, and advocacy.  Inherent in the role are the knowledge, skills, and abilities specific to health promotion, prevention, and protection.  The PHN effectively uses the nursing process to provide safe, ethical and client/family centered health care.  

The PHN role includes:
-Health assessment, health planning, health education, supportive counseling, preventative and screening measures, referrals and evaluations as appropriate
-Provides services to individuals, families, groups and communities with attention to diversity and the social determinants of health
-Immunization for all age groups, influenza program delivery, and health promotion initiative
-Communicable Disease Control management, including outbreak management, notifiable disease follow up
- Maternal/Infant health including postpartum support, strong focus on The Baby Friendly Initiative, lactation support and prenatal education, and high risk assessment of mother, babies, and families in the home environment
-Screens infants and preschool children for developmental milestones, identifies delays
