Job Description: Collaborate with schools and the community providing information and healthy lifestyles, facilitating positive lifestyle changes and healthy alternatives for youth. Job description is available upon request.

EDUCATION AND QUALIFICATIONS:

Graduate of a Registered Nursing Program or Bachelor of Science in Nursing.
Current CRNS registration.
Current CPR certification in Basic Life Support.
Maintains related CDM certifications such as Certified Diabetes Educator (CDE), Heart to Heart and living with stroke etc.
Current Drivers License

EXPERIENCE:

Requires a minimum of 3 years recent full-time clinical experience in specific CDM clinical area related to patients and families with specific needs (i.e. cardiology, diabetes, rheumatology and respiratory health
Requires minimum 1 year experience in patient/family/professional education.
Requires minimum 1 year experience in program/resource development related to needs assessment, planning, program design, implementation, and evaluation, teaching/counselling, facilitation skills, and leading group activities
